On the right side of the tank between the cage bottom and styrene. Right side meaning your right.

I would put the tank on some kind of raisers a few millimeters high as opposed to the styrofoam, because putting two soild surfaces together around a heat mat makes a fire hazard especially a highly flamable surface such as styrofoam. The best way to avoid a fire hazard with heat mats is to have a gap between the mat and the tank to stop it overheating a causing an electrical fire.

Make a cutout in the styrene foam around the heat pad. As for whether or not you need the foam, it depends how cold your room gets.
I have a BP in a Vision. In Winter, the foam on the bottom and back of the cage really helps in keeping the heat in. In Summer there is no need, but I leave it on anyway.

You mean so there is no styrene under the UTH at all, only around it? I thought the point of the styrene was to direct the heat upwards according to the Ultratherm install instructions.
